Calyxr (a product of Sunflower Lab LLC) operates as the Data Processor, managing technology services, messaging workflows, and communication platforms on behalf of Neuromonitoring Associates.

Neuromonitoring Associates values your privacy and is committed to protecting your personal health information and mobile contact details. This policy explains how NMA and Calyxr collect, use, and safeguard information obtained from patients through surgical intake consent forms, scheduling portals, and messaging interactions

We collect personal information necessary to deliver intraoperative neuromonitoring (IONM) care coordination and transactional updates, including:

• Patient Contact Details: Full Name, Mobile Phone Number, Email Address, and Mailing Address.
• Care & Surgical Details:Date of surgery, physician/surgeon information, and care status.
• Billing Information:Insurance carrier details, claim status, and payment communications.
• Messaging Records:Inbound/outbound SMS/RCS message history, opt-in records, and support inquiries.

Patient mobile numbers and messaging consents are collected via physical paper intake forms during surgical scheduling and registration. Patients review and execute the dedicated Text Messaging Consent clause on the Consent and Authorization for Intraoperative Neuromonitoring Services form prior to procedure date.

We do not sell, rent, or trade your personal information.
NO MOBILE INFORMATION WILL BE SHARED WITH THIRD PARTIES OR AFFILIATES FOR MARKETING OR PROMOTIONAL PURPOSES. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties under any circumstances.

Mobile numbers and personal details are strictly used to:

• Send procedure reminders and surgical coordination notifications.
