Neuromonitoring Associates — Terms of Service (Data Controller) Facilitated by Calyxr as Technology Provider.
This Terms of Service applies to mobile messaging and data collected on behalf of Neuromonitoring Associates LLC (“NMA”), who acts as the Data Controller. Calyxr (a product of Sunflower Lab LLC) operates as the Data Processor, managing technology services, messaging workflows, and communication platforms on behalf of Neuromonitoring Associates.
By providing your phone number and consenting to receive communications from NMA via signed intake forms or digital interactions, you agree to be bound by these Terms.

We collect personal information necessary to deliver healthcare notifications and IONM care coordination, including:
- Patient Name and Contact Details (Mobile Phone Number, Email Address).

NMA utilizes SMS, RCS, and automated communications powered by Calyxr AI to:
- Deliver critical appointment and surgical preparation reminders.
- Provide care instructions and educational video links (e.g., IONM information videos).
- Send billing, claim, and insurance updates.
- Deliver post-service satisfaction surveys.
- Respond directly to general patient inquiries or support requests.
4. SMS Privacy & Data Sharing (Carrier Compliance)
We value your privacy and do not sell your personal data.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
5. Message Frequency & Rates
- Message Frequency: Message frequency varies based on your surgical schedule, appointments, and billing updates
- Message & Data Rates: Standard message and data rates may apply from your wireless carrier depending on your mobile service plan.
6. Opt-Out and Help Instructions
- How to Opt Out: You can opt out of receiving text messages at any time by replying STOP, CANCEL, END, QUIT, STOPALL, or UNSUBSCRIBE to any message. Upon texting STOP, you will receive a final confirmation message confirming that you have been unsubscribed.
- How to Get Help: For support or assistance regarding messaging, reply HELP to any message, call our customer service team at 888-338-4270, or email customerservice@nmaiom.com.
7. Third-Party Links & Content
Messages sent by NMA may contain links to patient care videos, educational content, or portal updates (e.g., nmaiom.com). Our services may also link to parent platforms, including Calyxr AI and Sunflower Lab LLC. We encourage you to review the privacy policies of any third-party sites you visit.
